Writing dual-control entries that stay honest

Dual control fails quietly when the second signature becomes a courtesy. The fix is rarely a stern memo — it is a field design that forces a tiny observation.

Ask for something only a witness saw

Instead of “Witness: ______”, prompt for “Badge color of entrant” or “Container seal intact? Y/N”. Rubber stamps struggle when the line demands a detail.

Record late arrival truthfully

If the second person arrives after the door opens, write that. Inventing simultaneous presence helps nobody when cameras disagree. Exception language is healthier than fiction.

Rotate who holds the pen

When the same senior always signs as witness, juniors stop reading the entry. Swap roles during drills so both people practice noticing.
