Odd-hour openings are where tidy templates go to die. Alarms sound, escorts arrive early, and maintenance needs the room now. A playbook keeps the record honest when the script breaks.

Pre-write the awkward lines

Give staff three approved sentence stems: delayed witness, alarm-driven entry, and incomplete asset list. Choosing a stem is faster than inventing tone under stress.

Capture what you did not do

“Could not verify seal on container B before entry” is useful. Silence implies the seal was fine. Auditors prefer discomfort to fiction.

Close the loop next shift

Exceptions should trigger a next-shift acknowledgment, not vanish into the binder. A five-minute morning review prevents orphan notes.
